What to expect:

Initial Visit: 

Initial visits are 60-90 minutes in duration. Complete history, pertinent physical exams, review of past lab work or ordering of new lab work may be done. You will receive a personalized treatment plan at the end of your visit. Please bring past lab work or imaging to your appointment for the doctor to review. Bring a completed intake form to your appointment or arrive 10 minutes early to your appointment to fill out paperwork.

If you are using insurance for the appointment please check prior to the appointment if I am in network with your insurance by calling your company. Remember to bring your insurance card and ID. If it is an at time of service appointment fee schedule, payments can be made by check, venmo or square. 

 

Follow Up: 

Follow up appointments can be 30-60 minutes in duration and include medical history, re-evaluation, exams and treatment. Please notify the doctor if there are changes to insurance prior to your appointment. Please take time to understand your new insurance benefits if there have been changes prior to the appointment. Any questions regarding insurance billing can be directed to my medical biller at Thrive Practices. 

 

Online or phone consults also available:

Online or phone consultations provide you easy access to your doctor from the comfort of your own home. Your doctor can give you an individualized treatment plan to meet your health goals with your complete health history and labs.

Policies

These policies are set out to help keep things running smoothly for the practice and for patient care.

Cancellation Charge:

Please give 24 hour notice if you need to cancel or reschedule. There will be a $75 charge for late notice cancelations or missed appointments.

If there is an emergency that arises and you need to cancel in less than 24 hour notice time frame please call the office to notify. A credit card will need to be kept on file for no show appointments and for outstanding medical bills. 

 

Email:

In person office visit or Telehealth are preferred over portal messaging or email communication to discuss any new medical concerns. Please be aware that responding to messages takes time and professional expertise. Short and quick clarifications in treatment plans that can be answered within a few minutes are acceptable. If there is a need to access a patients chart, the questions is complex, lengthy emails, there is a need for new treatment or a change in patients treatment please schedule a follow up appointment. This is for optimal care of the patient and respect for the doctor.

Emails and portal messaging may be billed depending on time spent and complexity at a rate of $20-40 per email.

 

Emergencies:

For emergencies please call 911 or go to Urgent care. Email is not an appropriate form of communication for urgent questions or emergencies.

 

Insurance Billing:

Please refer to the numbers on the back of your insurance card if you have further questions or concerns.

Understanding your individual plan is important! Please be aware of your responsibility for remaining balances due to deductible or coinsurance. Insurance may not cover all fees. Final determination is made by your insurance company once the claim is received.
